Reason:
Conversion of 4-spans single phase primary line to 3-phase;
Erection of one 35ft and one 40ft concrete poles;

Replacement of one 35ft wood pole & Extension of 2-spans 3-phase primary line; and
Installation of 3-37.5 KVA private DT together w/ its accessories per Job Order No. 05-23-0040.
